As we close out the school year, I would like to share some important information from the health office. If your child requires medication during the 2025–2026 school year, the following action items are required:
- Completion of the Medication Administration Plan and Field Trip Consent Form
- A current medication order from a licensed prescriber
- All prescriptions must be provided in the original pharmacy-labeled container
- All medications must be dropped off by a parent or guardian
In addition, I encourage all parents/guardians to provide the health office with an updated physical exam and immunization record for the 2025–2026 school year.
Links for medication here:
Thank you for your attention to these important matters.
Please feel free to contact me with any questions or concerns,
Susan Williams MSN RN
